The “NEAR” DIABETES Experience
C. Norman Shealy, M.D., Ph.D.
Since “Near Death Experience” has become common, even with the current cover article in MISSOURI MEDICINE, it is time to become much more aware of the “Near Diabetes Experience.”
For many years I have been seeing patients with significant neuropathy but they have been assured by their physicians that they do not have diabetes—based just on a “normal” fasting blood sugar. Of course that “normal” fasting blood sugar might be just a bit above the lab range of up to 100 BUT there is significant evidence that a fasting blood sugar above 85 is already a trend that is likely to increase upwards. More importantly, the rarely done Hemoglobin A-1-C test is far more reliable.
Many of the patients whom I see with early neuropathy have a hemoglobin A-1-C of 6 or 6.1, which is pre-diabetes or metabolic syndrome. In other words, you are NEAR Diabetes! I was taught 58 years ago in medical school that diabetic neuropathy may develop up to 10 years before frank diabetes!
Frankly, if you are 25 pounds or more overweight, you are well on the way towards pre-diabetes. At least 90% of adult-onset diabetics are well overweight. Why wait for the “real” diabetes? Is “near” not good enough to do something NOW? Like stop wheat and carbs and add chromium and exercise!
